Vancouver Coastal Health’s Home Support offers many services. These can be provided on a short-term basis, such as after leaving the hospital, as part of end-of-life care, or over a longer period, such as several months or years.
Personal support services provided by community health workers (CHWs) include:
- Bathing, dressing, and toileting
- Helping clients move around or get in and out of bed safely
- Heating and serving meals
- Providing a break for caregivers
- Helping clients take their medication
- Connecting clients to a Social Worker who can check-in on you
Home Support Programs include: Home and Community Care, Meals on Wheels, Volunteer Drivers, Volunteer Shopping, Telephone Tree, Friendly Visitor, Seniors Outreach Counsellors and Shorncliffe Adult Day Center & Totem Lodge facilities.
